Having been involved in home nursing since 1992, Masako Akiyama has provided home end-of-life care to numerous people. Based on these experiences, she has come to realize the vital tasks in implementing home care and aging-in-place are (1) changing ways of thinking among the public; (2) changing ways of thinking among health care practitioners; (3) establishing a system of regional medical collaboration; (4) enhancing community nursing care systems; and(5) developing comprehensive administrative systems. The Kurashi-no-Hoken-shitu has served to accomplish tasks (1),(2) and (3) above, while lobbying for tasks (4) and(5),
